C & C Engineering has an LOTO Program outlining clear steps to performing the full LOTO procedure all the way from the shutdown of equipment to what to do if an employee leaves a lock.

Our training is structured to fully inform employees of the harmful effects of hazardous energy. Here are the four most common ways potentially fatal accidents happen during LOTO.

1. Full Procedure Uncompleted

A step by step procedure is thoroughly listed in our LOTO Program – all the way from the shut down of equipment to LOTO lock removal. If even one step is missed the repercussions can be fatal. For example the policy states that the system needs to be evaluated before shutting down. If an employee skips that step and a secondary generator was not found, that piece of machinery can turn on while someone is in the middle of repair. Our hands-on training focuses on following through with every step to inspect and evaluate machinery/equipment prior to shutting down!

2. Equipment/Tools Unaccounted For

Each employee is provided with a Lock Out Tag Out Kits. These kits are equipped with signs, tags, hasps, and locks. If even one piece is missing it could be catastrophic. We audit the kits to ensure that all locks are accounted for. Moreover, each kit includes personalized name tags that displays their name and picture. If a lock is left behind, the staff can track down who locked the equipment and make sure that employee is safe before engaging the machine.

3. Failure To Notify

Notify and inform all potentially affected area personnel, including contractors and customers of the up-coming shutdown and lock out. A failure to notify can mean grave danger. If not everyone on-site is aware of the shut down how can they be aware of the lock out.

When locking out equipment, a hasp and lock is placed on all points of energy device sources by all employees in the area. If even one employee is not aware of the shut-down they may reverse the lock out puttng a fellow employee at risk. Always notify personnel involved with the shut down and lock out.

4. Time Management

A full shut-down and lock-out can take anywhere from a couple hours to several days of lock out tag out procedure; so it is important for employees to understand that rushing is not an option. If an employee does not allow proper time allotted for the full procedure some steps may be skipped or proper placing of equipment can be forgotten.
